GENERAL REQUIREMENTS

1. Duly filled-up visa application form (signed)

2. Two passport sized colored picture with white back ground (must be taken within the last six months; do not staple the photos in the form)

3. Original passport (At least 6 months valid beyond intended stay)

4. Original old passport (if with previous travel or visa)

5. Copy of passport data page

6. Copy of roundtrip airline reservation

7. Copy of hotel reservation

8. Original personal bank certificate

9. Original bank statement or certified thru copy of passbook for the last 6 months

10. Copy of recent personal income tax return

11. PSA Birth certificate and/or marriage certificate (if travelling with immediate family)

SPECIAL REQUIREMENTS

FOR EMPLOYEES

• Original employment certificate with leave of absence (must include applicant’s position, date hired, compensation, office address and landline numbers and e-mail address)

• Copy of PRC/IBP ID (if professional)

• Copy of company ID

FOR BUSINESSMEN

• Copy of business registration (DTI or SEC)

• Copy of business permit or mayor’s permit

• Copy of income tax return

​

FOR STUDENTS/MINORS

• Original school certification

• Copy of School I.D.

• Copy of parent’s document such as original bank certificate, original COE or business documents

FOR RETIREE

• Copy of senior citizen card or retirement documents

​

IF THE COMPANY WILL SHOULDER THE TRIP

• Guarantee letter from company

• Original bank certificate and bank statements of the company (last 6 months)

• Copy of business documents and ITR of the company

All original documents must have a photocopy.

Note: additional documents may be required by the embassy without prior notice.
